Support Coordinator (SW)

DescriptionDevelops and completes participants care plan; establishes goals and directives of the care plans, and services to be utilized.
Maintains regular, ongoing contact with clients, caregivers, and physicians.
Problem solving skills by creating strategies to assist participants enhance their environment is a needed attribute.
This position is expected to possess the ability to assimilate their professional knowledge, and skills to create a person-centered plan of care while engaging participants with community resources when applicable.
Completes and submits timely paperwork and reports.
RequirementsEducation:
oLicensed Mastered or Bachelor prepared Social Worker.
Will accept limited licensed MSW or MSW for a maximum of (2) years after employment.
Experience:
expectationoTwo-three years of experience completing intakes, pre-screening, and/or assessments, preferably in a healthoKnowledge of Medicaid and Medicare regulations.
oAbility to complete assessments using computerized screening tools.
oExcellent Customer Service skills and ability to de-escalate callers.
Computer literate:
Word, Excel, OutlookoExcellent communication skills, written and verbaloCurrent Michigan driver's license and access to a car.
oAbility to analyze and problem solve.
Recommended Skills Assessments Communication Computer Literacy Customer Service Medicaid Medicare Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
