Nurse Practitioner Cardiology

Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a program, service, facility level, VISN, or VACO. The focus must be broad and complex and must be demonstrated at the macrosystem level. The expert nurse incorporates professional standards and implements broad-reaching evidence-based change. Leads and organizes delivery of services at the to assure continuity and peer accountability for practice. The position is in the Referral Coordination Center/Navigation Team. The incumbent will work with Medicine Service, Cardiology Clinic Section for their clinical practice. Incumbent is supervised by the Group Practice Manger (GPM). Duties but not limited to, Cardiology Clinic and the Referral Coordination Center (RCC)/Navigation Team Specific Scope of Practice:
a. The NP is responsible for planning, developing, implementing and evaluating the patient's medical and nursing care, collaborating with the supervising provider. b. Decide the appropriateness of cardiology consults c. Able to develop congestive heart failure patient's clinic and follow them on regular basis with appropriate management. d. Assist in the management of Pacemaker/A1CD patients e. Assess indications, contraindications and limitation of Exercise Stress Test and Pharmacological Stress Test and perform these tests independently with minimal supervision. f. ECG interpretation g. Telemetry - Rhythm recognition and treatment h. Demonstrates clinical knowledge with evidence-based practice, documentation, and accountability for practice i. Demonstrates interpersonal communication and collaboration skill j. Conducts self in professional manner. k. The NP will be dedicated to working primarily the RCC/Navigation Center and the Cardiology Clinic. l. NP will complete triage and complete various consults that are directed to the Referral Coordinate Team/Navigation Center for actions. m. NP will work closely with scheduling staff and provide clinic guidance in direction and support of scheduling activities to best meet the patient's needs. n. NP will work with patient and their families to provide care in the RCC/Navigation Center through face-to-face, video, telephone communications. The NP will assist the patient by providing them with the best options of reasons to ChooseVA vs. Care In The Community. This will involve direct communication and documentation through CPRS, VISTA, Email and potentially other systems of record. NP's work in the Referral Coordination Center will involve review of community care consults of all specialties, liaison with specialty clinic staff, identifying VA options for care, contacting Veterans and offering them VA options when available, forwarding community care consults to VA consults when appropriate and facilitate scheduling within the VA. NP will review Veteran eligibility for community care and approve care in the community when appropriate and forward consults to community care staff for scheduling in the community. o. NP will provide navigational routing of additional care to meet any prerequisites to full filling the consult request and continue to track patient until any necessary prerequisites may be full filled to meet Navigation Center and 91136-A patient needs. This may include additional placement of consults or orders in system and provide patient direction to meet the consult prerequisites pertaining to the RCC/Navigation Center. p. NP will be flexible and adaptable to changing environment within the Referral Coordinate Team/Navigation Center and work towards outcomes that improve the process for the employee and increase patient and employee satisfaction. This may include feedback, team meetings, and committee status and/or situational reports/updates. Practice:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Reference:

This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles. Telework maybe offered on a situational basis possible 25% of time.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
